Your feedback and insights will be used to develop our Disability Service Delivery Model, which is a document that helps guide our staff on how to provide a good service.

Conversation Details:
Conversations will be held over the phone or video call - whichever is easiest for you.
We can also arrange to meet in person if you’d prefer.
Surveys and conversations will take place in October 2025. You can opt out at any time.
Payment details:
This will be paid by bank transfer or gift card, you will get to choose. This is called an honorarium payment.
You may only complete one option and be paid once for your time.
We endeavour to make all payments within 2-4 weeks, however on occasion processing delays may occur.
