Referral Form

BSL Grow is a free, family-centered program, for families with children aged 2–3 years, providing family coaching, early learning support, and links to local services. Our goal is simple: to help your child learn, feel safe, and belong—while giving you the confidence and tools to create a strong future for your family.

Consent to collect your information and share it for your referral

To help connect you with the right supports, we need to collect some basic information about you and your family. We use this information to understand your needs, make referrals to other services (if you choose this), and keep a record in the Brotherhood of St. Laurence (BSL) system.

We may also contact you to check how your referral is going and may share relevant details within BSL teams so we can coordinate your support. We only share information when it helps us support you and always follow privacy laws and National Disability Insurance Agency (NDIA) requirements.

Your information will also help us improve BSL Grow. Sometimes we use de-identified information (so your name and details are not attached) for research, reporting, or to help improve programs.

You can change your mind at any time and tell us to stop contacting you or stop sharing your details within BSL, unless we are required to by law or need to ensure essential support continues.
